In the very early aughts I had an openwork locket intended to hold scented beads which would have accomplished this admirably.

Openwork, so not watertight, but there are poison-ring jewelry styles that do have tightly sealing compartments. Though I find rings less fidgety than pendant necklaces.

What does it say that I want a transparent glass bauble for the pills to go in. Make sure people around me understand what it says about *them* when the unused supply decreases as I spend time around them.

agreed. Can't help with the spare-pill part, though my friend Jack does do the jewelry/fidget part (look for OiShiny on Etsy if interested. UK-based, not sure current European shipping policies, think US off atm)
